Once you can see the preventive care activities recommended in your Care Checklist, you can complete them, submit proof, and earn points. This article walks you through how.
Completing a Preventive Care Activity
1. Select an Activity
Click on the preventive care activity you wish to complete from your checklist to view its details, including what’s required to complete the activity.
2. Visit Your Physician / Required Lab
Visit your physician or the required lab to complete the preventive care activity or undergo the specified screening, and collect valid proof of completion.
Note: You do not need to complete this step if you have already completed the preventive care activity during the specified Program Duration.
3. Submit Your Activity for Review
Admin approval is generally required for preventive care activities, but this may vary based on your employer’s Reward Program settings. Some employers may choose not to require proof or admin approval.
Completion Date: Entering the date you completed the activity is mandatory.
Proof of Completion: If required by your employer, upload valid documentation as proof of completion.
Accepted proof formats:
.pdf,.jpg,.jpeg,.png,.heic
Maximum file size: 10 MBComments: Add any relevant comments or additional information, if applicable.
Once submitted, the activity will be sent for admin review, if review is required.
4. Wait for Admin Review and Approval
Your submission is sent to your administrator for review.
Once it's reviewed and approved, you'll earn the points associated with that activity.
Understanding Frequency
Some preventive care activities can be completed more than once during the program duration. For example, a Dental Exam may be completed twice a year.
The frequency and number of eligible completions are configured by your employer as part of the Reward Program. Each activity will display its applicable frequency, so you’ll know how often you can complete it and earn points during the program duration.
