Your Health Risk Assessment is a confidential, evidence-based questionnaire that evaluates your health across 13 categories, giving you a clear, personalized picture of where you stand and where small changes can make the biggest difference.
Once submitted, you'll receive your risk scores along with tailored recommendations to help you improve.
Taking Your Health Risk Assessment
1. Go to the Health Risk Assessment
From the main menu, go to Health → Health Assessment.
2. Review the Privacy Policy and Consent
Before you begin, review the privacy policy shown on screen and provide your consent to continue. Your responses are confidential and protected.
3. Complete the Questionnaire
Work through the questionnaire, which covers a broad range of health and lifestyle categories to build a complete view of your wellbeing. This typically takes about 10 minutes.
4. Complete or Skip the Biometrics Section
When you reach the biometrics section, you can choose to complete it or skip it. This section is entirely optional. Adding your biometric values makes your results more precise, but skipping it will not prevent you from finishing or from earning any points your employer has configured.
5. Submit Your Assessment
Once you've answered all the questions, submit your assessment. Your results are generated immediately.
Can I Earn Points for Completing My Health Risk Assessment?
Yes! Completing your Health Risk Assessment (HRA) may earn you points, depending on how your employer has configured your wellness program.
If your program offers points for completing the HRA:
You’ll receive the applicable points after completing the assessment.
You can still earn the points even if you skip the optional biometrics section.
Point values and eligibility requirements are determined by your employer.
For specific point values and program requirements, check your Rewards Program or contact your program administrator.

Refresh the page and make sure you've accepted the privacy policy to continue
Complete any required questions before submitting
Log out and log back in, then return to Health → Health Risk Assessment
