My Care Checklist is a personalized set of preventive care activities recommended just for you. By sharing a few basic details, you'll receive tailored recommendations for screenings and check-ups that help you stay on top of your health.
Accessing Your Care Checklist
1. Open Your Care Checklist
From the Main Menu, click Health, then select My Care Checklist.
2. Enter Your Date of Birth and Gender
Click Provide Details, then enter your date of birth and gender. Wellness360 uses this information to generate personalized preventive care recommendations.
Note: If your employer has already provided these details, you will automatically skip this step.
3. View Your Personalized Recommendations
Once submitted, you'll see your personalized list of preventive care activities to complete. Each recommendation is tailored to you based on the details you provided.
Important: Only the activities incentivized by your employer will earn you points upon completion. Other activities may still be shown as part of your personalized recommendations, but completing them will not earn you any points.
