Once you've completed your Health Risk Assessment, Wellness360 translates your answers into clear, actionable results. This article walks you through how to read your scores, get personalized recommendations, and download your report.
Reviewing Your Results
1. View Your Risk Scores
After submitting, you'll see your overall results along with a category-by-category risk breakdown (Low / Moderate / High). Each category reflects an area of your health, so you can quickly see your strengths and the area's most worth your attention.
2. Get Personalized Recommendations
Every category includes personalized Recommendations based on your results. Click View Recommendations to explore the resources selected for you.
Daily Habits - small, repeatable actions that build healthier routines over time
Journeys - guided, multi-step programs focused on a specific goal
Courses - structured learning to deepen your understanding
3. Enroll to Improve Your Score
You can enroll directly from the recommendations, so improving your score is as simple as choosing where to start.
4. View Trends and Comparison
If this is not your first time completing the HRA, your results page also shows trends and comparisons with your previous HRA, helping you track how your health is changing over time, not just where you are today. Click “vs Last HRA” to view your comparison.
5. Download Your Results
You can download your HRA report at any time using the Download Results Report button at the top of the page. Just select which assessment (by date) you'd like to download.
Having Trouble Viewing Your Results?
If your results or recommendations aren't displaying correctly, try the following:
Refresh the page and reopen your results
Confirm your assessment was fully submitted
Log out and log back in, then return to Health → Health Risk Assessment
