How Can I View and Analyze My Health Risk Levels?

  • Upon completing the Health Risk Assessment (HRA), a Wellness Score will be generated that gives you an overall insight into your health and wellness quotient.

  • The risk levels for each assessment parameter are given on the Health Assessment page and also on the Risk Advisor page. Based on the outcomes, you may be categorized into - Low Risk, Moderate Risk, and High-risk levels.

  • A brief description of the specific parameter, its impact on your health and wellness, along with the recommended suggestions are displayed for each parameter record.

  • The Assessment Results, relevant Risk Advisor levels, and recommended suggestions can be viewed on the Wellness360 web platform or on the Wellness360 mobile app.

From the Wellness360 web platform:

1. Log in to the Wellness360 web platform using the correct credentials.

2. Navigate to the Health Section from the top menu bar. The Health Risk Levels can be viewed from the Health Assessment page or the Risk Advisor page.

Health Assessment Page:

  • The risk levels for all the assessment parameters are displayed with a simple graphical representation. Based on the assessment input, the risk levels will be grouped in low, moderate, and high-risk levels.

  • A brief of the assessment parameter, its risk level description is shown when we scroll down, and the recommendations are displayed for each record.

  • Clicking on ‘Recommendations’ opens a slide-bar with the suggested preventive measures and recommended healthy habits and Journeys programs to reduce the risk levels of that specific parameter.

From the Wellness360 app:

1. Install the Wellness360 app and log in with the correct credentials.

2. Navigate to the ‘Health’ section from the bottom toolbar to take the assessment.

3. Once the assessment is completed successfully, you will receive a Wellness Score and overall health risk level, displayed above the Health map.

4. Tapping on any parameter like Nutrition, Exercise, and others, on the map, will redirect you to a page with the risk level results of the specific parameter and the recommended suggestions.
