Overview and Status of Physician Forms
Accessing Physician Forms
Navigate to the Reports section in the Wellness360 Admin platform.
From the available list, click on Health Reports.
In the dropdown list displayed, click on Physician Form.
You will now see an overview displaying the total number of submitted forms along with their statuses.
Below the overview, the Physician Form Report will be available, listing individual form submissions.
Filtering by Date Range
This page includes a Date range filter, allowing you to narrow down records from the last month to the last year.
Use this feature to refine results based on specific timeframes.
Viewing a Specific User's Status and Details
Locate the user entry for which you wish to review the physician form.
Under the Actions column, click on the three horizontal dots icon for that entry.
A small pop-up will appear with the option View Details.
Click on View Details to open a pop-up window displaying the Physician Form Status and information about the submitted form.
By following these steps, you can efficiently access and review physician form details and statuses submitted by users, ensuring seamless health data management within your organization.
